YSB's ultimate focus is to ensure all youth have access to safety, shelter and the opportunity to succeed. We do this through offering an array of services including a transitional living program for pregnant/parenting young women. We work to help youth repair foundations which have been jolted by abuse, neglect, homelessness and lack of resources, by guiding youth into safety, ensuring they have shelter, and teaching them how to overcome various obstacles. Youth Service Bureau delivers client-centered, trauma-responsive, and preventative services and support to youth experiencing safety and housing crises.

What You’ll Do:

As the Porchlight Case Manager, you will provide young mothers with intensive case management sessions using Trauma-Informed Care, Positive Youth Development, and Evidence-Based Practices. Your responsibilities will include helping youth develop self-sufficiency plans, coordinating and leading daily life skills groups, and offering resources on parenting and child development. You will maintain a client-centered and solutions-based approach when working with clients. Additionally, you will plan and facilitate outings for residents.

Your Accountabilities in the Role:

Client Engagement & Assessment

  • Conduct thorough, trauma-informed intake and assessment of pregnant and/or parenting youth, young adults, and their children to identify strengths, needs, and risk factors.
  • Develop individualized service plans in partnership with clients that reflect goals in housing stability, education/employment, parenting skills, health, and social supports.

Case Planning & Coordination

  • Create and manage service and case plans that define goals, interventions, timelines, and measurable outcomes.
  • Coordinate access to services, including prenatal/postnatal care, mental health and substance use services, education programs, employment support, and child development resources.
  • Facilitate linkage to mainstream and community resources and ensure referrals follow-up through regular client contact.

Housing & Safe Exits

  • Work directly with clients to identify safe, appropriate, and stable housing options during program participation and at exit.
  • Provide coaching on independent living skills — such as budgeting, money management, tenant responsibilities, and daily living competencies — to prepare participants for long-term stability.
  • Support youth in transitioning from MGH to permanent or supportive living situations, including family reunification, independent living, and housing assistance programs.

Education & Economic Stability Support

  • Assist youth with school enrollment, GED attainment, vocational training, employment readiness, job search support, and career planning.
  • Monitor and document client participation and progress toward educational and employment goals.

Parenting & Child Development Services

  • Provide or coordinate parenting education, and other life skills, that promotes the young adult’s social emotional development, well-being, and daily life skills, as well as their child(s) health, safety, attachment, and developmental milestones.
  • Link clients to early childhood education, childcare, and other family supports to optimize child well-being and client independence.

Documentation & Data Reporting

  • Maintain accurate, timely documentation in the HMIS system and other required reporting mechanisms to track progress toward performance standards (e.g., housing outcomes, employment, education, health).
  • Compile and submit performance data to support compliance with funding requirements and grant reporting timelines.

Trauma-Informed & Culturally Competent Practice

  • Deliver services using a trauma-informed, strengths-based approach that respects cultural, linguistic, and developmental diversity.
  • Identify and respond to signs of trauma, human trafficking, exploitation, and other complex issues affecting clients and their children.

Advocacy & Client Support

  • Advocate on behalf of clients with service providers, schools, landlords, employers, and social service agencies.
  • Provide crisis intervention and support during emergencies, safety planning, and linkage to medical or legal services when necessary.

Collaboration & Community Engagement

  • Build and maintain effective partnerships with community providers, health care agencies, housing authorities, educational institutions, and workforce programs that can strengthen service access for MGH clients.
  • Participate in community meetings and interagency collaborations to enhance referral networks and program visibility.
  • Plan and facilitate social activities, events, and outings for MGH clients.

Facilities and Environment of Care

  • Conduct routine environment of care, unit, and safety checks to ensure living spaces are safe, sanitary, and compliant with agency, licensing, and FYSB MGH requirements.
  • Identify, document, and report facility and safety concerns, submitting maintenance requests as needed and escalating urgent issues to ensure timely resolution.
  • Collaborate with residential staff to maintain a trauma-informed, child-safe, and welcoming environment, reinforcing expectations around cleanliness, shared space use, and program safety.

Quality Assurance & Professional Development

  • Maintain compliance with all grant conditions, agency policies, and FYSB guidelines.
  • Attend ongoing training on best practices, trauma-informed care, human trafficking, and youth development to maintain competencies. This includes mandatory program and agency staff meetings.

Position Requirements:

· Education: Bachelor’s degree preferred in social work, psychology, or a related field.

· Experience: 2 years of social work experience, preferred.

·Technology Skills: Basic computer skills, Office 365.

·Communication Skills: Must possess excellent written and verbal communication skills.

· Leadership/ Behaviors: Integrate trauma-informed care, positive youth development, and transition to independence skills into all client interactions while maintaining ethical boundaries and professionalism. Must be self-sufficient without requiring constant supervision

Other Important Information:

Pay/Salary: $18 to $20 per hour, DOE

Reports To: TLP Supervisor

Core Hours: Wednesday through Sunday, 12 pm-8pm

Typical Work Week: Wednesday through Sunday

Travel: As needed for work-specific trainings and client needs.
