This is an office-based position, focused on case management, service coordination, documentation, and collaboration with families and providers.
What Our Services Look Like
We provide assistance to individuals and families through the coordination of services and supports that allow adults to continue living successfully in their family home.
Our services include:
Multilingual support — We speak English, Spanish, and Tagalog, with additional languages continually being added
Developing independent living skills within the home
Coordinating and ensuring services are delivered effectively
Providing information and resources related to the individual’s diagnosis and identified supports
Assisting with scheduling service delivery, including medical and other appointments
Identifying transportation options and coordinating access to services
Supporting future planning and long-term goal development
Assisting individuals in accessing and engaging with their community

Position Overview
The CFS Specialist provides comprehensive case management services in coordination with the Regional Center, individuals served, families, and community providers. This role ensures services are effectively implemented, progress is monitored, and documentation meets regulatory and agency standards.
The ideal candidate is organized, detail-oriented, and passionate about supporting individuals and families through structured coordination and advocacy.
Key Responsibilities
Manage and coordinate an assigned caseload in collaboration with the Regional Center
Communicate regularly with individuals served, families, and service providers
Maintain accurate, timely, and compliant case documentation and progress notes
Prepare reports, assessments, and required documentation according to agency and Regional Center standards
Monitor service delivery to ensure alignment with Individual Program Plan (IPP) and assessment goals
Participate in interdisciplinary meetings and case reviews
Maintain strict confidentiality and compliance with all applicable regulations and agency policies
Qualifications
Bachelor’s degree in Social Work, Psychology, Human Services, or related field (preferred)
Experience in case management, social services, or working with individuals with developmental disabilities (preferred)
Strong organizational, time management, and documentation skills
Professional written and verbal communication abilities
Ability to manage multiple priorities and meet deadlines
Proficiency in Microsoft Office and electronic documentation systems
